Today on 26th December, 2022 Orchid Medical Centre hosted a press conference on the successful treatment of Brain Aneurysm of a 44-year-old patient. This press conference was addressed by Dr P. K. Gupta- Chief Administrative Officer, Dr Vikram Singh – Consultant Neuro Surgeon, M. Ch, Neurosurgery (NIMHANS), Dr Amit Kumar Gupta – Consultant Anesthetist.
Brain aneurysms are a weakness in the arteries of the brain that swell and fill with blood. When such aneurysms rupture, it can lead to life threatening bleeding called subarachnoid haemorrhage. Cases of poor grade subarachnoid haemorrhage carry a mortality rate of 50-70 percent. The outcomes are even worse in elderly patients. Most such patients seek treatment in metro cities, leading to loss of valuable time for the patient.
Recently, a similar case was treated at Department of neurosurgery, Orchid Medical centre. a 44yr old lady was brought to the emergency in an unconscious state. Her CT brain showed subarachnoid hemorrhage with hydrocephalus. An emergency surgery was done at midnight by placing an external ventricular drain to relieve the hydrocephalus. The next day, the patient became conscious and alert. A digital subtraction angiogram was done which revealed an aneurysm arising from the supraclinoid segment of left internal carotid artery. The aneurysm was not suitable for clipping. Hence, coiling of the aneurysm was done on Dec 13, 2022. After the coiling was done, attempts were made to block the external ventricular drain (EVD) but the patient was not able to tolerate EVD closure. In view of this, a ventriculoperitoneal shunt was done. The patient has recovered well after the surgery and is conscious and alert.
WFNS grade IV subarachnoid hemorrhage (SAH) has an unfavourable outcome rate approaching 60%. Those who survive have high rates of disability. We are glad to report this case of Grade IV SAH where the patient could be saved with excellent functional outcome.
